The unique anatomical structure of the feline ear canal—with its L-shaped curvature and deep, narrow vertical canal—combined with the warm, moist microenvironment within the canal means that once the skin barrier is compromised or immune defenses are weakened, fungi seize the opportunity, causing otitis externa and potentially spreading to the middle ear. Establishing standardized end-to-end operating procedures, from precise diagnosis to standardized medication and relapse prevention, is the sole reliable pathway to eradicating ear canal fungal infections.
Precise diagnosis is the starting point of the standardized protocol. Symptoms of ear canal fungal infection include frequent head shaking and ear scratching, a sour or yeasty odor emanating from the ear canal, erythema and swelling of the pinna and canal skin, and increased secretions—with Malassezia infection typically presenting as dark brown, waxy discharge and Candida infection more commonly as creamy white-yellow discharge. However, visual inspection and odor assessment alone are easily confounded with bacterial infections and ear mites; cytological examination of ear canal secretions is mandatory. Procedure: collect ear canal secretions with a cotton swab, smear evenly onto a glass slide, perform Diff-Quik rapid staining, and examine under a microscope. Malassezia organisms appear as characteristic "peanut" or "footprint"-shaped budding spores against a background of abundant keratinized epithelial cells; bacterial infections show numerous neutrophils with intracellular bacteria. For recurrent or treatment-refractory cases, submitting samples for fungal culture and antifungal susceptibility testing is recommended to identify the species and guide precise pharmacotherapy.
Ear canal cleaning is the prerequisite for successful pharmacotherapy. Secretions and inflammatory debris not only obstruct contact between the medication and the lesion site but also serve as a culture medium for fungal proliferation; they must be thoroughly removed before each dose. Feline-specific ear cleaning solutions containing squalene, salicylic acid, or lactic acid, with pH controlled in the mildly acidic range of 3.5-4.5—an environment that itself inhibits Malassezia growth—are recommended. Antifungal drug selection should be precisely matched based on cytology or culture results. First-line agents for Malassezia infection include miconazole (2% concentration), clotrimazole (1% concentration), and ketoconazole (2% concentration) otic drops. Combination otic preparations—such as commercial formulations containing neomycin, nystatin, and triamcinolone—offer antifungal, antibacterial, and anti-inflammatory effects, suitable for mixed-infection cases. Application technique directly influences drug distribution within the ear canal and therapeutic efficacy: ensure the ear canal is cleaned and dried before instillation, deposit the drops deep into the vertical ear canal, immobilize the cat's head, immediately massage the ear base for 1-2 minutes—hearing a "squelching" sound indicates even drug distribution throughout the canal. The medication should remain in the ear canal for at least 5-10 minutes, after which the cat may be allowed to shake out excess. Administer 1-2 times daily for at least 2 weeks until symptoms resolve, followed by an additional week of consolidation therapy; the total treatment course is generally 3-4 weeks, with premature discontinuation being the most common cause of relapse.
Oral antifungal medications are indicated for severe or refractory ear canal fungal infections and cases that have extended to the middle ear. Itraconazole is the preferred oral antifungal agent for cats, dosed at 5-10 mg/kg daily for 2-4 weeks. Due to its hepatotoxic potential, serum liver enzyme levels (ALT, AST, ALP) should be measured before treatment and rechecked after 2 weeks. Ketoconazole has been progressively supplanted by itraconazole due to its more pronounced hepatotoxicity and poor feline tolerance. Terbinafine (30-40 mg/kg daily) demonstrates potent fungicidal activity against Malassezia and serves as a second-line or combination agent for refractory Malassezia otitis. All oral antifungal agents must be used under veterinary prescription and monitoring; self-procurement and self-administration are strictly prohibited.
Relapse prevention strategies center on three pillars: dryness, acidity, and barrier integrity. Maintaining a dry ear canal is the core of fungal relapse prevention: insert cotton balls into the ear canal before each bath to prevent water ingress, and after bathing, blot visible external ear moisture with dry cotton balls. For relapse-prone cats that enjoy swimming or live in high-humidity environments, a weekly application of an ear canal drying powder containing desiccant ingredients such as diatomaceous earth micropowder may be used. Concurrently, periodic use of a mildly acidic ear canal care solution (pH 3.5-4.5) maintains the acidic barrier of the ear canal. For cats with confirmed underlying allergic conditions such as atopic dermatitis or food allergy, ear canal fungal infections are often secondary manifestations; the primary allergic disease must be concurrently managed or recurrence is inevitable. Allergy management includes food allergen investigation, omega-3 fatty acid supplementation (EPA+DHA 30-50 mg/kg daily), and, when necessary, immunomodulatory therapy.
Differentiating ear mites from fungal infections is a common clinical challenge, compounded by the fact that mixed infections frequently occur. The characteristic secretion of ear mite (Otodectes cynotis) infestation is dry, dark-brown, coffee-ground-like debris, whereas Malassezia infection produces moist, oily, dark-brown waxy discharge. Under microscopy, adult ear mites appear ovoid with four pairs of legs extending beyond the body margin, a morphology distinctly different from Malassezia budding spores. In terms of treatment strategy, mite-fungal mixed infections should follow a mite-first-then-fungus or concurrent approach: single-dose administration of macrocyclic lactone antiparasitics such as selamectin or doramectin effectively eradicates ear mites; once mites are controlled, the fungal infection often improves accordingly. In cases of isolated ear mite infestation, prolonged indiscriminate use of antifungal otic preparations must be avoided—it is ineffective and disrupts the normal ear canal microbiome.

复发的预防策略聚焦于"干燥+酸性+屏障"三要素。保持耳道干燥是预防真菌复发的核心:每次洗澡前在耳道塞入棉花防止进水,洗后用干燥棉球吸干外耳可见水分。对于易复发的游泳爱好者猫或高湿度环境生活猫,可每周使用一次含干燥剂成分(如硅藻土微粉)的耳道干燥粉。同时,定期使用弱酸性耳道护理液(pH3.5-4.5)维持耳道酸性屏障。对于已确诊为特应性皮炎或食物过敏等基础过敏性疾病的患猫,耳道真菌感染往往是其继发表现,必须同步管理原发过敏性疾病,否则复发难以避免。过敏管理包括食物过敏原排查、Omega-3脂肪酸补充(每日EPA+DHA 30-50mg/kg)和必要的免疫调节治疗。
耳螨与真菌感染的鉴别是临床常见难点,且两者经常混合感染。耳螨(耳痒螨)感染的特征性分泌物为干燥、黑褐色咖啡渣样碎屑,而马拉色菌感染为湿润、油性的深褐色蜡样分泌物。显微镜下耳螨成虫呈椭圆形、四对足且超出体缘,与马拉色菌芽生孢子形态截然不同。治疗策略上,若螨菌混合感染应先杀螨后抗真菌或同步进行:塞拉菌素或多拉菌素等大环内酯类驱虫药单次使用即可有效杀灭耳螨,待螨虫控制后真菌感染常随之改善。若耳螨单独感染切勿长期滥用抗真菌耳药,这不仅无效还会破坏耳道正常菌群。
